File A Claim
You have two options for filing a claim for money from the Settlement:
1) submit the Claim Form online
2) print the Claim Form and submit it by U.S. Mail

If you submit your claim online: ALL REQUIRED FIELDS MUST BE COMPLETED BEFORE THE FORM IS SUBMITTED, INCLUDING THE CERTIFICATION AND SIGNATURE PAGE. YOUR CLAIM WILL NOT BE SUBMITTED UNTIL AFTER YOU ELECTRONICALLY SIGN THE CLAIM AND PRESS THE BLUE “SUBMIT” BUTTON.
An acknowledgement will appear on your computer screen in a few seconds after pressing the submit button. Please print for your records as it includes your claim number.
To submit your claim via email or U.S. Mail please use the link above to print a hard copy version of the Claim Form. You must complete the Claim From and return it to the Settlement Administrator no later than August 1, 2022. The Claim Form must be postmarked and mailed to the Settlement Administrator by August 1, 2022, to:
1) submit the Claim Form online
2) print the Claim Form and submit it by U.S. Mail
